Yes, it does occur at the edge of your network (on your DSL modem for instance).

No, there's no evidence to suggest that it happens all throughout the Internet.

Yes, it's a problem, and kudos to Jim for bringing attention to it.  When it comes to congestion-management solutions I think there's still a lot more room for debate, but first people have to admit that huge buffers just make things worse.

Yes. It screws up the flow control that TCP depends on. Initially I was sceptical, but once I worked a few numbers, by golly, he's right.
