A jpeg parser, running on a surveillance camera, which crashed every time the company's CEO came into the room. 100% reproducible error.
Reason was: JPEG parser choked when the CEO came into the room, because he always had a shirt with a square pattern on it, which triggered some special case of contrast and block boundary algorithms. :-)
