Well the Hulk 2003 wasn't quite popular and earned mere $245.3 million as to a budget of $137 million. Also it was produced by shared studios(Universal+Marvel) that means Marvel wasn't yet established. Going through all this, they then recast Edward Norton for their own The Incredible Hulk 2008  under Marvel Studios banner. the movie goes well( same as of Hulk but critics praised the character).
But then Edward Norton shows no more interest in playing The Green Giant anymore. He feels he isn't of that type.
So Marvel again had to find a new face, and then they catch Mark Ruffalo. He was quite much praised and I hope he'll last a lot longer.

Whenever actors work in a sequel movies, the producers get a bond signed from them stating that they will work till the end of the sequel. Maybe they weren't planning on expanding the hulk universe back then.

Example:
For Iron Man, Robert Downey Jr. Signed a bond for 15 years from Marvel. So whenever marvel releases a new iron man movie or other movies like "Avengers" featuring Iron Man he got to play the part. That's why his role is consistent in Marvel Universe.

Simple

First Actor was cast for the first movie.
The first movie flopped, and when the series was rebooted, a new actor was cast.
For Avengers, the same actor as the second movie was supposed to be cast (Edward Norton), but he said he wanted more diversity in his carrier and did not want to repeat the same role again. So, the third actor (Mark Ruffalo) was cast. He is contracted to be the Hulk in i think 5 or 6 more movies.
